Description

The health services that became theAnthony L Jordan Health Corporation (http://www.jordanhealth.org/) (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants ofRochester, NY (http://www.city-data.com/city/Rochester-New-York.html) and in Canandaigua, N.Y.

Jordan Health is now seeking anAdolescent Medicine Physicianwho, under the guidance of the Chief of Pediatrics, is primarily responsible for providing high-quality comprehensive medical care to pediatric and teen patients at Jordan Health. Medical care includes a holistic view, including accessing social and behavioral care within our health system and beyond.

Requirements

TheAdolescent Medicine Physicianwill ensure that:

· Clinical outcomes are improved through evidence-based strategies and innovative care approaches.

· The Adolescent Medicine Physician contributes to the organization’s aspiration towards health equity.

· Organizational systems and processes support Jordan Health’s strategic direction and operational expectations are achieved.

Education and Experience Required:

· Adolescent Fellowship or at least 5 years work with pediatric-teen populations.

Licenses and Certifications:

· Board eligibility or certification in pediatrics required.

· Fellowship training desired.

· Must possess a current NYS license as a MD or DO at the time of start.

· Must have a DEA at the time of start.

Special Skills, Knowledge Required:

· Ability to work well with adolescent patients.

· Ability to deliver care through health promotion teaching or counseling, technical expertise, crisis intervention and evaluation of the client’s response to interventions.

· Cognizance of the prevalent stressors and their impact in both the professional provider and the patient populations in the community health center primary care setting.

· Ability to work creatively with other health professionals from a variety of disciplines to achieve maximum productivity.

· Ability to assist in promoting growth and development of fellow professionals.

About Jordan Health:

Jordan Health is an independent FQHC, with Level III Patient-Centered Medical Home (PCMH) designation through the National Committee on Quality Assurance. Jordan Health receives funding from the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services. Jordan Health is a network of outpatient primary care offices with providers who follow a panel of patients. While independent, Jordan Health actively collaborates with the major hospital and healthcare systems in our operating area to provide a total safety net of healthcare services.

Salary Description

$180,000-$200,000/ANNUALLY
